Individual Registration

Individual Registration Form  We will assign your child to a team in the same age group and ability (Rep/House League) Teammate Request: if your child wants to play with a friend, make sure you indicate the friends name on the registration under Teammate Request Indicate if your child wants to play up a level in the notes section of the registration Please provide the players birthdate and current level of hockey Team Code: If you have been assigned to a team, and been provided with a code, you MUST enter the team code on the registration  Payment: Here, you will complete the registration online and pay ONLY by credit card

Team Registration

Team Registration Form Register a team from our registration page to reserve a spot for your team in our 4on4 league We will issue you a team code and instructions for your players on registration information U11 4on4 Division: 12 players and 1 goalie U13/U15/U18 3on3 Division: 10 players and 1 goalie Coaches are responsible for filling up there team's roster, if you are short players, please notify the league ASAP as we have individual players registering. Keep in mind we will fill up the individual teams first, any excess players will be moved to team entries that are short on players.
